Medical Plans and District Benefits Stipend Allotment for 2011-12

Effective October 1, 2011, the parties agree:
- 1. That the District will continue to provide the same medical plans listed in 14.3.1.
- 2. The following changes will be made to prescription drug coverage:
- a. Prescription drug coverage for the following plans shall be provided through Medco:
- i. Blue Shield PPO High Option Plan
- ii. Blue Shield PPO Medium Option Plan
- iii. Blue Shield HMO High Option Plan
- iv. Blue Shield HMO Low Option Plan
Note: Generic drugs for all of these plans will be free at Costco (no membership required to participate).
- b. Prescription drug coverage for the Blue Shield PPO Low Option Plan shall continue to be provided by Blue Shield in accordance with the plan design.
- c. Co-payments for prescription drug coverage for the Blue Shield HMO Plans will change as follows:
- i. Blue Shield HMO Low Option Plan:
- 1. 30-day supply
Current: $10/$20/$35 for Generic/Brand/Non-Formulary
Renewal: $9/$35 for Generic/Brand - 2. 90-day mail-in supply
Current: $20/$40/$70 for Generic/Brand/Non-Formulary
Renewal: $18/$90 for Generic/Brand
- ii. Blue Shield HMO High Option Plan:
- 1. 30-day supply
Current: $5/$10/$25 for Generic/Brand/Non-Formulary
Renewal: $5/$ 10 for Generic/Brand - 2. 90-day mail-in supply
Current: $10/$20/$50 for Generic/Brand/Non-Formulary
Renewal: $10/$ 20 for Generic/Brand
- 3. The benefit stipend amounts set forth in section 14.2 shall be adjusted to reflect the applicable new rates as follows:
- a. 14.2.1 Medical Coverage for Employee Only:
The District shall provide full-time regular and contract unit members with medical coverage for employee only, a benefits stipend for medical, dental, life insurance and disability insurance, up to $9,018.84 per year, calculated based on their assignment status. - b. 14.2.2 Medical Coverage for Employee Plus One Dependent:
The District shall provide full-time regular and contract unit members with employee plus one dependent in medical coverage, a benefits stipend for medical, dental, life insurance and disability insurance, up to $17,516.16 per year, calculated based on their assignment status. - c. 14.2.3 Medical Coverage for Employee Plus Two Dependents:
The District shall provide full-time regular and contract unit members with employee plus two dependents in medical coverage a benefits stipend for medical, dental, life insurance and disability insurance up to $24,136.44 per year, calculated based on their assignment status. - Date: __________ For the Cabrillo Community College District By: __________
