- CCFT and the District agree to the following in regards to the Managed Health Network (MHN) mental health benefit of the Health Net HMO High Option Plan:
- Effective October 1, 2007 through September 30, 2008, the MHN mental health benefit for faculty members on the Health Net HMO High Option Plan will return to a $10 co-pay with unlimited visits.
- Faculty members who have paid the higher co-pay of $30 for the MHN benefit between July 1, 2007 and September 30, 2007 will be reimbursed by the District for the difference of $20 per visit. In order to request a reimbursement, faculty members must provide a copy of the Explanation of Benefit Payment form (EOB) from MHN that outlines the services rendered and reflects the $30 co-pay for each visit, to Susan Torres in the Human Resources Department.
